Job description
CVS Health is hiring a Nurse Practitioner or Physician Assistant to conduct in-home health assessments for Medicare Advantage members in the Grand Rapids, MI area. This is a part-time, field-based role where you travel to members' homes to perform comprehensive assessments, including physical exams, medication review, and health risk assessments. No facility-based patient care. Flexible schedule, competitive pay.
Key responsibilities

What you bring
- Active NP or PA license in Michigan
- Ability to travel within Grand Rapids area
