Job description
Provide In-Home Health Evaluations for health plan members, including medication and medical history review, physical evaluation, and diagnostic tests as ordered. Use iPad and clinical workflow. No prescribing or altering treatment regimens. Travel locally within licensed states. Conduct virtual visits as needed. Full-time position with comprehensive benefits.
Key responsibilities
- Conduct in-home health evaluations (approx. 45 minutes)
- Perform medication and medical history review
- Perform physical evaluation and diagnostic tests (e.g., diabetic eye exams, spirometry)
- Use iPad and clinical workflow for each evaluation
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
What you bring
- Active unrestricted NP or PA license in coverage area
- Board certification
- Multi-state licenses preferred
- Comfortable working in diverse home environments
