Job description
Conduct in-home health evaluations for health plan members, including medication and medical history review, physical evaluation, and diagnostic tests as ordered. Use iPad and clinical workflow. No prescribing or altering treatment regimens. Travel locally and within licensed states. Full-time role with benefits.
Key responsibilities
- Perform in-home health evaluations (approx. 45 min each)
- Review medication and medical history
- Conduct physical evaluation and diagnostic tests (e.g., diabetic eye exams, spirometry)
- Use iPad and clinical workflow for documentation
- Travel locally and within licensed states as needed
- Conduct virtual visits as needed
What you bring
- Active unrestricted NP or PA license in coverage area
- Board certification
- Multi-state licenses preferred
