Job description
Provide In-Home Health Evaluations, engaging with people in their homes to bridge gaps in care. Conduct 45-minute visits including medication and medical history review, physical evaluation, and diagnostic tests as ordered. Use iPad and clinical workflow. No prescribing or lab orders required. Travel locally and within licensed states as needed. Conduct virtual visits as needed.
