In-Home Health - Nurse Practitioner or Physician Assistant (Per Diem) - Wenatchee/ Grant County, WA

Provide In-Home Health Evaluations for health plan members in their homes. Each visit lasts about 45 minutes and includes medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., diabetic eye exams, spirometry). Use an iPad and clinical workflow; no prescribing or lab orders required. Travel locally within licensed states; conduct virtual visits as needed. Must be board certified and have active unrestricted license in coverage area (Washington). Multi-state licenses preferred. Per diem role with hourly base rate plus per-visit incentives.
