Conduct in-home health risk assessments for Medicare and Medicaid members in the Bronx, NY. Use clinical judgment to identify gaps in care, coordinate with primary care providers, and educate members on chronic condition management. Flexible part-time schedule, mileage reimbursement, and competitive hourly rate.

Key responsibilities

  • Perform comprehensive in-home health assessments

What you bring

  • Active NP or PA license in New York
  • Strong clinical assessment skills
  • Ability to travel within Bronx
  • Experience with geriatric or underserved populations preferred
