The Integrated Care Clinical Manager at CareSource/Commonwealth Care Alliance is a community-based RN responsible for providing monitoring, follow-up, and clinical care management to dually-eligible enrollees with complex medical, behavioral, and social needs. This role focuses on integrating health services and community resources to coordinate enrollee care. Essential functions include engaging with enrollees in their homes and community settings, performing assessments (Comprehensive Assessment, MDS-HC Functional Assessments, Crisis and Risk Assessments), developing care plans, leading interdisciplinary care teams, overseeing long-term services and supports, assisting with community resources, educating members about benefits and chronic condition management, following up after hospitalizations, coordinating with healthcare providers and community organizations, and advocating for enrollees. Requires an Associate's degree in nursing, an unrestricted RN license in Massachusetts, a valid driver's license and vehicle, and willingness to travel over 50% of the time. Influenza vaccination is required. Salary range: $90,000 - $120,000.

Key responsibilities

  • Engage with enrollees in homes and community settings to establish care management relationships
  • Perform comprehensive, functional, and risk assessments
  • Develop and implement care plans with enrollee input
  • Lead interdisciplinary care team and collaborate with internal/external partners
  • Oversee utilization of long-term services and supports
  • Assist members in accessing community resources (housing, transportation, food, social services)
  • Educate members about Medicare/Medicaid benefits and chronic condition management
  • Follow up after hospitalizations or significant health events
  • Coordinate with healthcare providers and community organizations
  • Advocate for enrollee needs and preferences

What you bring

  • Active unrestricted RN license in Massachusetts
  • Associate's degree in nursing
  • Valid driver's license and vehicle
  • Intermediate proficiency in Microsoft Office (Outlook, Word, Excel)
  • Knowledge of Medicare and Medicaid programs
  • Strong interpersonal and communication skills
  • Case management experience preferred
  • Case Management Certification highly preferred
