Hybrid Registered Nurse (RN) Clinical Care Manager - Chicopee - 5K Sign On Bonus

The Clinical Care Manager is responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical and psychosocial needs of the member to facilitate and support the member's overall wellness. This role involves developing a plan of care jointly with the member, family, and physician, coordinating services, and monitoring outcomes. The position is hybrid, requiring in-office work in Chicopee, MA, and remote work. No direct patient care is performed; the role is desk-based and telephonic. Requires an active, unrestricted RN license in Massachusetts, 2 years of clinical experience, and case management experience preferred. Salary range: $60,000 - $90,000 annually.
