Job description
The Care Manager is responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical and psychosocial needs of the member. This role works in a hybrid capacity, with some days in the Brooklyn office and some days remote. The Care Manager will coordinate care for members with complex health needs, collaborate with providers, and ensure appropriate utilization of resources. Requires an active RN license in New York, 2+ years of clinical experience, and strong communication skills. BSN preferred. Salary range: $65,000 - $95,000.
