Job description
CVS Health is hiring a Case Manager RN for LTSS (Long-Term Services and Supports) in Wayne and Macomb County, Michigan. This is a field-based role requiring travel to members' homes to conduct assessments, develop care plans, and coordinate services. No in-person patient care at a clinical facility.
