Conduct virtual home evaluations for patients in their homes. This is a field-based role where you travel to patients' homes to perform assessments via telehealth technology. Requires active NP or PA license in Utah. Must have reliable transportation and be willing to travel within assigned territory. No in-person patient care at a clinical facility.
