Job description
Deliver patient care services through a remote technology platform as a Virtual Care Nurse Practitioner. Provide holistic, evidence-based care including assessment, diagnosis, treatment, health counseling, and disposition planning for patients aged 18 months and above. Document encounters in Epic. Must hold current unrestricted NP license in Tennessee and live within three hours of Nashville or Knoxville. Minimum 2 years medically-relevant experience. Per diem requirement: minimum 9 hours availability per week, evening shift 2:30p-11p.
Key responsibilities
- Provide holistic, evidence-based care via virtual platform
- Evaluate primary care, acute, chronic, and transitional care patients
- Provide patient counseling including pregnancy prevention and STI prevention
- Document all patient care in EHR per policies
- Coordinate care with internal and external colleagues
- Work independently and as part of a patient care team
What you bring
- Active NP license in Tennessee
- National Board Certification as FNP
- 2+ years medically-relevant experience
- Proficient with EHR and technology
- Effective communication and organizational skills
