Job description

Optum Home & Community Care seeks a part-time Telephonic Nurse Practitioner for after-hours call coverage (evenings, nights, rotating weekends, holidays). Provide telephonic/telehealth care and direction to patients, caregivers, and facility staff. Responsibilities include managing medical and behavioral conditions, performing comprehensive assessments, coordinating care transitions, and documenting in EMR. Requires active NP license in one of: NY, NJ, CT, ME, NH, or RI, plus ability to obtain in other locations. National board certification (FNP, ANP, GNP, AGACNP) and DEA licensure required. Minimum 20 hours/week, every other weekend, holiday rotation. Sign-on bonus up to $30,000.

Key responsibilities

  • Provide telephonic/telehealth care to members 24/7
  • Perform comprehensive assessments and document findings
  • Coordinate care across settings and transitions
  • Manage acute and chronic conditions using evidence-based guidelines
  • Participate in quality initiatives and mandatory training

What you bring

  • Active NP license in NY, NJ, CT, ME, NH, or RI
  • National board certification (FNP, ANP, GNP, AGACNP)
  • DEA licensure or ability to obtain
  • Proficient with EMR and telephonic platforms
  • 3+ years experience preferred
