Job description

Devoted Health is looking for a Telehealth Preventative Primary Care Advanced Practice Provider (NP/PA) to join our team. In this role, you will provide high-quality, patient-centered primary care via telehealth, focusing on preventive care and chronic disease management. You will work remotely, conducting virtual visits, reviewing patient histories, ordering labs and diagnostics, and collaborating with a multidisciplinary care team. This role requires an active NP or PA license, with preference for experience in primary care or geriatrics. The position is 100% remote, with a salary range of $115,000–$130,000.

Key responsibilities

  • Conduct virtual primary care visits with a focus on prevention and chronic disease management
  • Review patient histories, order labs and diagnostics, and develop care plans
  • Collaborate with physicians, nurses, and care coordinators to ensure comprehensive care

What you bring

  • Active NP or PA license in any US state
  • 2+ years of clinical experience in primary care or geriatrics
  • Comfort with telehealth technology and EMR systems
