Job description
CVS Health is hiring a Registered Nurse Case Manager for field-based work in Monmouth and surrounding counties in New Jersey. This role involves conducting in-home assessments for members with complex health needs, developing care plans, coordinating with providers, and educating members on disease management. Requires active RN license in New Jersey, 3+ years of clinical experience, and strong communication skills. BSN preferred. Mileage reimbursement provided. Salary range: $75,000–$95,000.
