Job description
The Care Manager is responsible for managing a caseload of members in a Managed Long Term Care (MLTC) plan. This hybrid role requires working from a Manhattan office 2-3 days per week and conducting field visits to members' homes in the five boroughs. Duties include comprehensive assessment, care planning, coordination of services, and monitoring member outcomes. Must have an active NY RN license and 2+ years of experience in home care, long-term care, or case management. BSN preferred. Salary range: $75,000 - $95,000.
