Field Nurse Practitioner (Spokane, WA)

Job description

Provides screening, preventive primary care and medical care services to members primarily in non-clinical settings including in-home, community, nursing facilities, and pop-up clinics. Performs comprehensive evaluations, chronic and acute care, care coordination, post-discharge follow-up, and community outreach. Drives up to 120 miles daily within assigned region. Requires active FNP license in Washington, DEA license, BLS, and valid driver's license.

Key responsibilities

  • Provide general medical care and care coordination to various member populations
  • Perform comprehensive evaluations including history and physical exams for gaps in care
  • Address chronic and acute primary care complaints, ascertain medical urgency
  • Order diagnostic tests judiciously and create medical plans of care
  • Perform face-to-face in-person visits in homes, skilled nursing facilities, and public locations
  • Participate in community-based pop-up clinics
  • Drive up to 120 miles daily; may require extended mileage and overnight stays
  • Collaborate with fellow NPs and participate in regional meetings
  • Document in EMR and prescribe medications as appropriate

What you bring

  • Active and unrestricted FNP license in Washington
  • Active DEA license
  • BLS certification
  • 1+ year NP experience
  • Valid driver's license and reliable transportation
  • EMR proficiency
  • Strong communication and time-management skills
