Field Care Manager, LTSS (RN) - Local Travel Required

Job description

Texas licensed RN to join Molina as a Field Care Manager working with Medicaid members in the Tyler, TX area. Complete assessments for waiver member eligibility, develop care plans, coordinate services, and monitor outcomes. Requires in-person home visits and local travel (25-40%). Mileage reimbursed. Hours Monday-Friday 8am-5pm CST. Requires 2+ years healthcare experience including 1 year in care management/managed care and 1 year with LTSS or equivalent. Active Texas RN license, valid driver's license, reliable transportation, and auto insurance required. BSN may be required in some states. CCM preferred.

Key responsibilities

  • Complete comprehensive member assessments within regulated timelines, including in-person home visits
  • Facilitate waiver enrollment and disenrollment processes
  • Develop and implement care plans in collaboration with members, caregivers, and healthcare professionals
  • Perform ongoing monitoring of care plan effectiveness and document interventions
  • Promote integration of services including behavioral health and LTSS
  • Assess medical necessity and authorize waiver services
  • Facilitate interdisciplinary care team meetings
  • Use motivational interviewing to support member change
  • Assess barriers to care and coordinate assistance
  • Identify critical incidents and develop prevention plans

What you bring

  • Active Texas RN license
  • 2+ years healthcare experience
  • 1+ year care management/managed care experience
  • 1+ year LTSS experience
  • Valid driver's license and reliable transportation
  • Microsoft Office proficiency
  • Knowledge of community resources
  • Strong communication and problem-solving skills
