Job description

As the Wavier Case Manager, you are the key player in integrating Case Management and Social Services for clients in Kentucky's waiver program. You lead a dynamic, collaborative process to assess, plan, implement, coordinate, monitor, and evaluate services, ensuring they meet your client's health care needs. You are their advocate, championing their well-being while educating staff on resource utilization and discharge planning. Your role is vital in making a real difference in their lives!

Primary Responsibilities

  • Client Needs Assessment and Coordination: Identifying client needs and ensuring they are met through effective coordination and monitoring of services
  • Service Planning and Implementation: Leading the person-centered service planning team and overseeing the development, implementation, and updates to care plans
  • Quality and Compliance: Assessing the quality, safety, and cost-effectiveness of services, ensuring compliance with Medicaid Waiver requirements, and maintaining accurate documentation
  • Client Advocacy: Acting as a patient advocate and communicating effectively to ensure the best interest of the client
  • Education and Training: Educating clients and staff on case management processes, rights, risks, responsibilities, and available resources
  • Collaboration: Coordinating with various service providers, agencies, and departments to ensure comprehensive care and resolve eligibility issues

Preferred Qualifications

  • Bachelor's or Master's degree in a health and human service field with 1+ year of experience in geriatric or disabilities field OR Current, unrestricted Kentucky RN license with 2 years of experience in geriatric or disabilities field
  • Proven broad knowledge base of special needs groups, community groups, and service organizations

The salary for this role will range from $53,064 to $79,596 annually based on full-time employment.
