Title: Telehealth Nurse Practitioner, Advanced Practice Provider Location: Remote
Role Description
The purpose of a Nurse Practitioner at Oak Street Health is to provide effective and equitable value-based primary care to adults on medicare to keep them happy, healthy, and out of the hospital. Nurse Practitioners see patients independently and collaborate with physicians, depending on the patient's specific situation.
Our Telehealth program offers care to our patients in the comfort of their own home and creates access for patients who need follow up appointments. We are looking for Nurse Practitioners to complete same day sick visits, triage of acute escalations, post-discharge visits, chronic disease management and annual wellness assessments for our patients via Telemedicine (in particular, via video/phone technology).
Central Telehealth Nurse Practitioners practice in our virtual care practice with a collaborative care team composed of a medical assistant and a dedicated medical scribe for documentation. We focus on episodic care in close partnership with the patient's primary care provider to enable optimal implementation of our care model. This role reports to the Central Telehealth Medical Director and works closely with operational leadership.
Core Responsibilities
- Utilize evidence based practice to provide best in class virtual care
- Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation
- Visits via video and phone
- Triage acute escalations from call center and support next steps in management as appropriate
- Care coordination with a multidisciplinary team including: Primary care providers, licensed medical social workers, behavioral health specialist, specialty care, testing facilities
- Utilize a population health focus to track patient outcomes and collaborate with providers in regular outcome reviews
- Caregiver and patient education on preventative care, lifestyle modifications and self management strategies
- Demonstrate a nimble approach and a willingness to adjust to the evolving and growing nature of the telehealth program as a key enabler of Oak Street's care model
- Willingness to obtain and maintain multiple state licenses (RN, NP, CSR, DEA) according to business needs
- Other duties as assigned
Required Qualifications
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification in at least one of the following specialties:
- Family Nurse Practitioner
- Adult-Gerontology Primary Care Nurse Practitioner
- Adult Nurse Practitioner
- Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license with the ability to obtain new licenses as needed
- US Work Authorization
- Someone who embodies being "Oaky"
Preferred Qualifications
- 3-5 years of experience in primary care: internal medicine, geriatrics, or family medicine
- 1-2 years of virtual care experience
- Licensure in the following states strongly preferred: AL, AR, AZ, CO, IA, IL, IN, LA, NM, OK, TN, TX
- Willingness to work across various time zones: MT, ET, CT
- Spanish language skills strongly preferred
- Proficiency in using telehealth platforms and technology
- Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).
