RN Case Manager Field Based--Hot Springs or Garland County Arkansas

Optum Home & Community Care seeks an RN Case Manager for field-based work in Hot Springs or Garland County, Arkansas. The RN performs in-home visits, telephonic outreach, and interdisciplinary team activities to manage health problems and coordinate care for patients. Responsibilities include assessing health status, developing and implementing care plans, evaluating patient responses, collaborating with a multidisciplinary team (NP, Behavioral Health Advocate, Care Navigator), consulting with PCPs and specialists, facilitating transitions of care, documenting in EMR, and providing patient/family education. Requires current unrestricted RN licensure in Arkansas, 2+ years RN experience, experience assessing patients with complex needs, computer literacy, ability to work with diverse care teams, and ability to travel up to 75% with a valid driver's license. Preferred: home health or geriatric experience, EMR skills, strong communication and organizational skills. Salary range $60,200 - $107,400 annually.
