Job description
Provides care management and care coordination for long-term services and supports (LTSS) for high-need members. Completes comprehensive member assessments via in-person home visits. Facilitates waiver enrollment/disenrollment, develops care plans, monitors effectiveness, and promotes integration of services including behavioral health and community resources. Assesses medical necessity and authorizes waiver services. Facilitates interdisciplinary care team meetings. Uses motivational interviewing to educate and support members. Assesses barriers to care and coordinates assistance. Identifies critical incidents and develops prevention plans. May conduct medication reconciliation. Requires 25-40% local travel in Massachusetts (Cambridge, Somerville, Brighton, Allston, Waltham, Arlington, and possibly Dorchester, Roxbury, Mattapan).
Key responsibilities
- Complete comprehensive member assessments within timelines, including in-person home visits
- Facilitate waiver enrollment and disenrollment processes
- Develop and implement care plans in collaboration with members, caregivers, and healthcare professionals
- Monitor care plan effectiveness and document interventions
- Promote integration of services including behavioral health and LTSS
- Assess medical necessity and authorize waiver services
- Evaluate covered benefits and advise on funding sources
- Facilitate interdisciplinary care team meetings
- Use motivational interviewing to educate and support members
- Assess barriers to care and provide coordination
What you bring
- Active and unrestricted RN license in Massachusetts
- 2+ years healthcare experience including 1 year in care management/managed care/medical/behavioral health setting
- 1+ year experience with persons with disabilities, chronic conditions, substance abuse disorders, and LTSS
- Valid driver's license, reliable transportation, and auto insurance
- Knowledge of community resources
- Proactive and detail-oriented
- Ability to work independently and with diverse populations
- Excellent time-management and prioritization skills
- Strong verbal and written communication skills
- Microsoft Office proficiency
