Nurse Case Manager

CarepathRx supports health systems and hospitals with pharmacy and infusion services designed to improve patient care.

We are seeking a Case Management Analyst (RN) to coordinate admissions and ongoing care for patients receiving infusion therapy. You will work closely with patients, providers, pharmacists, home health agencies, and internal teams.

Responsibilities

• Coordinate patient referrals and admissions.
• Confirm patients meet admission and clinical requirements.
• Communicate with patients, providers, pharmacists, home health agencies, and internal teams.
• Provide ongoing case management, including help with infusion pumps, supplies, and care transitions.
• Educate patients and caregivers about therapy needs and available resources.
• Deliver responsive customer service and help improve team processes.

Required Qualifications

• Graduate of an accredited nursing program.
• Active Registered Nurse (RN) license in PA or Compact.
• Strong communication, organization, and time management skills.
• Proficiency with Microsoft Office.
• Ability to manage multiple priorities in a fast-paced environment.

Preferred Qualifications

• Bachelor of Science in Nursing (BSN).
• Experience in home infusion, specialty pharmacy, case management, or home health.

If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.

For this position, we anticipate offering an hourly rate of 24 - 37 USD / hourly, depending on relevant factors, including experience and geographic location.

This role is also anticipated to be eligible to participate in an annual bonus plan.
