Job description
The Director of Case Management/Behavioral Health Coordinator is responsible for clinical operations and medical management activities across the continuum of care for enrollees in the Arizona Long Term Care System (ALTCS). This includes case management, coordination of care, problem-solving at member, family, and provider levels, and medical/behavioral health management consulting. The role also involves providing health education, coaching, review/approval of service plans, and leading projects. This hybrid position requires two to three in-office days per week in Phoenix, Arizona, and about 25% travel within assigned regions. Key responsibilities include monitoring court-ordered treatment processes, facilitating Child and Family Team meetings, supervising LTC Coordinator for the Office of Human Rights, overseeing transitions from RBHA to LTC, developing annual goals to reduce medical expenses, training staff and providers on behavioral health services, and ensuring contractual adherence through monitoring, auditing, and reporting.

- Monitor Court Ordered Treatment process for accuracy and timeliness
- Facilitate quarterly Clinical Connection meetings for licensed LTC staff
- Supervise LTC Coordinator for Office of Human Rights
- Oversee transitions from RBHA into LTC for members with SMI designation
- Develop annual goals to reduce medical expenses and improve quality of care
- Provide training to case management staff and providers on behavioral health services
- Oversee High Risk Behavioral Health case management staff
- Serve as key behavioral health resource and consultant
- Lead LTC case management department and ensure contractual adherence
- Review program descriptions and consult with potential providers
What you bring
- Current unrestricted Licensed Behavioral Health Professional in Arizona
- 5+ years serving elderly or individuals with physical/developmental disabilities and SED/SMI
- 5+ years management experience
- Supervisory and training experience
- Basic knowledge of ALTCS/Medicare/Medicaid regulations
- Proficient in Microsoft Word, Excel, PowerPoint
- Located in Maricopa County, AZ
- Ability to travel 25% in assigned regions
- Valid driver's license and reliable transportation
