Job description
Field-based case manager RN conducting in-home assessments and care coordination for members in Morris County, NJ. Develop and implement care plans, coordinate with healthcare providers, and ensure members receive appropriate services. Requires active NJ RN license, 3+ years clinical experience, and reliable transportation. Mileage reimbursement provided.
Key responsibilities
- Conduct in-home assessments of members' health status and needs
- Develop and implement individualized care plans
- Coordinate with physicians, specialists, and community resources
- Monitor member progress and adjust care plans as needed
- Document all interactions and interventions in electronic health record
What you bring
- Active New Jersey RN license
