Job description
This is a field-based role covering Cumberland and Atlantic counties in New Jersey. The Case Manager RN conducts in-home assessments and develops care plans for members with complex health needs. Responsibilities include coordinating care, educating members on disease management, and collaborating with healthcare providers. Requires active RN license in New Jersey, 3+ years of clinical experience, and strong communication skills. BSN preferred. Mileage reimbursement provided.
Key responsibilities
- Conduct in-home assessments of members
- Develop and implement individualized care plans

- Educate members on disease management and wellness
- Document all interactions in the electronic health record
