Care Manager, LTSS (RN) - Allen County

Job description

Provides care management and care coordination for long-term services and supports (LTSS) members. Completes comprehensive member assessments via in-person home visits, develops care plans, monitors progress, authorizes waiver services, and facilitates interdisciplinary team meetings. Requires active RN license, 2+ years healthcare experience including 1 year in care management and 1 year with LTSS/disabilities. Local travel 25-40%. Pay range $26.41-$51.49/hour.

Key responsibilities

  • Complete comprehensive member assessments within regulated timelines, including in-person home visits
  • Facilitate waiver enrollment and disenrollment processes
  • Develop and implement care plans in collaboration with members, caregivers, and providers
  • Monitor care plan effectiveness and document interventions
  • Promote integration of services including behavioral health and community resources
  • Assess medical necessity and authorize waiver services
  • Facilitate interdisciplinary care team meetings
  • Use motivational interviewing to support member behavior change
  • Assess barriers to care and coordinate assistance
  • Identify critical incidents and develop prevention plans

What you bring

  • Active RN license in state of practice
  • 2+ years healthcare experience
  • 1+ year care management or managed care experience
  • 1+ year experience with LTSS, disabilities, chronic conditions
  • Valid driver's license and reliable transportation
  • Knowledge of community resources
  • Strong communication and problem-solving skills
  • Microsoft Office proficiency
