Job description
Provide virtual primary care and chronic condition management to patients via telehealth platform. Conduct comprehensive assessments, develop care plans, prescribe medications, and coordinate with interdisciplinary team. Must be bilingual (English/Spanish). Active NP license required. 100% remote position with flexible scheduling.
Key responsibilities
- Conduct virtual patient visits for primary care and chronic disease management
- Develop and implement individualized care plans
- Prescribe medications and order diagnostic tests as needed
- Collaborate with care team including RNs, social workers, and specialists
- Document encounters in EHR system
What you bring
- Active Nurse Practitioner license in any US state
- Bilingual (English/Spanish) required
- 2+ years NP experience, preferably in primary care or telehealth
- Proficiency with telehealth technology and EHR
- Strong communication and patient engagement skills
