Job description
The Behavioral Health Case Manager manages and coordinates case management services for members with complex needs. Responsibilities include member assessment across behavioral, physical, functional, financial, and psychosocial domains; developing individualized care plans; collaborating with members, families, and providers; supporting behavioral health services and substance use disorder treatment; providing advocacy and education; meeting with members via telephone or in the community (home, provider office, community settings, or inpatient facilities); documenting activities per NCQA/Medicaid standards; understanding insurance benefits and regulations; and handling inbound/outbound calls. Requires current unrestricted LCSW, RN, LMFT, or CPC license in Nevada and 2+ years behavioral health experience. Must be available to travel up to 25% in Clark or Washoe County, NV. Salary range $60,200 - $107,400.
