Job description
Devoted Health is seeking an Advanced Practice Provider (NP or PA) to join our Palliative Care team. This is a remote role providing virtual palliative care consultations to patients. Responsibilities include conducting comprehensive assessments, developing care plans, managing symptoms, and coordinating with the interdisciplinary team. Must have active NP or PA license and experience in palliative or hospice care.
Key responsibilities
- Conduct virtual comprehensive assessments for palliative care patients
- Develop and implement individualized care plans
- Manage symptoms and provide medication management
- Collaborate with interdisciplinary team including physicians, social workers, and chaplains
- Educate patients and families on disease progression and goals of care
What you bring
- Active NP or PA license
- Experience in palliative or hospice care
- Strong communication and telehealth skills
- Ability to work independently in a remote setting
