Job description

Conduct in-home health assessments for Medicare and Medicaid members in New Jersey and surrounding markets. Use clinical judgment to identify gaps in care, coordinate with primary care providers, and educate members on chronic condition management. Travel required within assigned region; mileage reimbursed. Must have active NP or PA license in New Jersey. 1+ year experience as NP/PA. Bilingual Spanish preferred.
