Job description
Provide In-Home Health Evaluations for health plan members, including medication and medical history review, physical evaluation, and diagnostic tests as ordered. Use iPad and clinical workflow. No prescribing or altering treatment regimens. Travel locally within licensed states. Conduct virtual visits as needed. Per diem role with hourly base rate plus incentive structure.
Key responsibilities
- Perform in-home health evaluations (approx. 45 min each)
- Review medication and medical history
- Conduct physical evaluation and diagnostic tests (e.g., diabetic eye exams, spirometry)
- Use iPad for clinical workflow
- Travel locally and within licensed states
- Conduct virtual visits as needed
