Provide In-Home Health Evaluations for health plan members, engaging with people in their homes to bridge gaps in care. The visit lasts about 45 minutes and includes medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., diabetic eye exams, spirometry). Use an iPad and clinical workflow; no prescribing or lab orders required. Travel locally and within licensed states as needed; conduct virtual visits as needed. Part-time role with select benefits and malpractice insurance.
