Job description

Provide In-Home Health Evaluations, engaging with people in the comfort of their own homes. The visit lasts about 45 minutes and includes medication and medical history review, physical evaluation, and additional diagnostic tests if ordered. Use an iPad and clinical workflow. Not required to prescribe medicine, order lab tests, or alter treatment regimen. Travel locally and within licensed states as needed. Conduct virtual visits as needed. Active unrestricted license in coverage area required. Board certification required. Multi-state licenses preferred.

Key responsibilities

  • Conduct in-home health evaluations including medication review, physical exam, and diagnostic tests

What you bring

  • Active unrestricted NP or PA license in Minnesota
  • Board certification
  • Completion of 2080 hours post-graduate collaborative practice in integrated clinical care setting
  • Comfortable working in diverse home environments
