Job description

Perform care management for complex, high-acuity long-term care members. Develop and continuously update personalized care/service plans, coordinate medical and support services across the continuum of care, and educate members and caregivers on benefits and community resources.

Assess service needs of high-risk members and recommend plans for best outcomes. Collaborate with providers, specialists, and community resources to address member needs. Monitor member status for changes in condition, complications, and potential need for higher levels of care or waiver eligibility. Review referrals and intake assessments to build appropriate care plans. Coordinate service authorizations for meals, transportation, housing, ADLs, and similar supports.

Act as liaison and advocate between members, families, physicians, and facilities/agencies. Document all care management activities in compliance with state, federal, and clinical guidelines. May precept and coach new clinical hires through onboarding and competency checkpoints.

May perform home and/or site visits (approximately monthly) to assess member needs. Travel up to 5%. Position is designated for use by Arizona, Superior (TX), and Sunflower (KS) plans only.

Requires graduation from an accredited school of nursing or a Bachelor's degree with 4–6 years of related experience. BSN preferred. Active RN (state or compact) or NP licensure required. Superior plan requires RUG certification within 90 days of hire. Pay range: $36.21–$65.09/hour.

What you bring

  • Active RN license (state or compact) or NP licensure
  • 4–6 years of related clinical/care management experience
  • LTSS / long-term care assessment experience
  • Care plan development and documentation
  • Knowledge of community resources and waiver programs
  • RUG certification (Superior plan, within 90 days)
