Bilingual RN Care Manager

Remote, telephonic care management role supporting members with chronic conditions. Must reside in Puerto Rico and be fully bilingual in English/Spanish (proficiency testing required in both). The RN Care Manager assesses members' physical, environmental, and psycho-social needs, develops and manages care plans, resolves barriers to care, and guides members toward appropriate resources. Members typically have chronic conditions including diabetes, hypertension, COPD, and chronic kidney disease, with a Medicare population focus. Work involves telephonic outreach, ongoing assessments, and coordination with internal teams under general guidelines with minimal direct supervision. Schedule: 8:30 AM–5:30 PM EST, 40 hours/week, with occasional overtime or weekend coverage as business needs require. Occasional travel to Humana offices for training or meetings may be required. Required: BSN, active unrestricted RN license in both Puerto Rico and Florida, CPEPR affiliation, bilingual English/Spanish with proficiency testing, and prior clinical experience in adult acute care, skilled nursing, rehab, or discharge planning. Preferred: health plan experience, prior case management, call center/triage experience, and Medicare member experience. Salary: $56,900–$78,200 plus bonus eligibility. Home office requires minimum 25 Mbps down / 10 Mbps up internet and a private, HIPAA-compliant workspace.
