Certified Diabetes Education Specialist - Registered Nurse

The Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation and evaluation to engage, educate, and promote member decisions related to achieving optimal health status. Assessment of members through clinical tools and data review, comprehensive evaluation of member's needs and benefit plan eligibility. Utilizes assessment techniques to determine health literacy, technology capabilities, and readiness to change. Enhances medical appropriateness and quality of care by applying criteria and guidelines. Uses holistic approach, consults with supervisors and Medical Directors to overcome barriers. Utilizes motivational interviewing to ensure member engagement and promote lifestyle/behavior changes. Provides coaching and information to empower members. Interprets clinical guidelines to positively impact health. Helps members participate in healthcare decision-making. Develops and monitors plans of care in collaboration with member and physician. Documents in compliance with regulatory guidelines. Must be an RN and Certified Diabetes Care and Education Specialist (CDCES). Active unrestricted license in state of residence with multi-state/compact privileges. 3+ years clinical nursing experience post licensure. Experience with diabetes patients. Must have high-speed internet.
