Job description
Field-based RN Case Manager role at Oak Street Health primary care clinics in central Pennsylvania (Bethlehem, Allentown, Harrisburg). Partners with PCPs to lead care management for complex Medicare patients, focusing on admission prevention, readmissions, and transitions of care.
Key responsibilities
- Manage caseload of complex value-based care patients
- Lead transitions of care including post-discharge assessments and medication reconciliation
- Coordinate with PCPs, social workers, and behavioral health specialists
- Lead in-person interdisciplinary care planning meetings
- Perform nursing assessments and patient education
What you bring
- Active PA RN license
- 6-8 years nursing experience
- CCM certification (or within 12 months)
