Job description
The Health Coach Consultant uses clinical judgment to provide telephonic coaching and support to members with chronic conditions. Responsibilities include assessing member needs, developing care plans, educating on disease management, and coordinating care. This is a 100% remote role requiring an active RN license and 3+ years of medical-surgical or related experience. BSN preferred. Salary range: $70,000–$90,000.
Key responsibilities
- Conduct telephonic health coaching sessions with members
- Assess member health status and develop individualized care plans
- Provide education on chronic disease management and medication adherence
- Coordinate with healthcare providers and community resources
