Clinical Call Center Triage Nurse

The Clinical Call Center Triage Nurse extends care during and outside of regular Oak Street Health clinic hours by addressing patients' medical, social, and psychological needs telephonically. You'll provide confident, professional service to OSH patients, follow prescription refill policy, and document clearly and precisely during calls. Responsibilities include managing the CareReq queue, scheduling off-cadence appointments, arranging transportation for clinic visits, and meeting departmental metrics including average handle time, calls per hour, and quality metrics.

A structured career path exists from Triage RN I to Triage RN II based on tenure, problem-solving, and performance metrics.

Remote work requirements include proficient PC skills, basic Google Suite, prior remote work experience, ability to hardwire to router/modem, and a distraction-free private workspace with reliable dependent care during working hours.

Requirements: Active, non-probationary state RN license in all states of OSH practice; ability to work autonomously; flexibility to work shifts outside core business hours including evenings, weekends, and holidays; US work authorization. Pay range $29.10–$62.32/hour, eligible for bonus/incentive program.
