Job description
The CDCES Health Coach Consultant provides telephonic health coaching to members with diabetes and other chronic conditions. Uses motivational interviewing and evidence-based guidelines to support self-management goals. Collaborates with interdisciplinary team to coordinate care. Requires active RN license and CDCES certification. 3+ years of diabetes education or case management experience preferred. Remote position with occasional travel for team meetings.
Key responsibilities
- Conduct telephonic health coaching sessions with members
- Develop personalized care plans using motivational interviewing
- Monitor member progress and adjust interventions as needed
- Collaborate with physicians, pharmacists, and other team members
- Document interactions in electronic health record
