Job description
CVS Health is hiring a Field Case Manager RN for Mercer County, NJ. This role involves conducting in-home assessments and coordinating care for members with complex health needs. You will work collaboratively with members, families, and healthcare providers to develop and implement care plans. Requires an active RN license in New Jersey, 3+ years of clinical experience, and strong assessment and communication skills. BSN preferred. Must have reliable transportation and be willing to travel within Mercer County.
