Job description
CVS Health is hiring a Field Case Manager RN to conduct in-home assessments and coordinate care for members in Essex and surrounding counties within 50 miles of Newark, NJ. This role involves traveling to members' homes to evaluate health status, develop care plans, and connect members with community resources. No in-person patient care at a clinical facility. Requires active RN license in New Jersey, 3+ years of clinical experience, and reliable transportation. Salary range: $70,000–$90,000.
Key responsibilities
