Job description
The Case Manager RN uses clinical judgment to coordinate care for members, conducting telephonic assessments, developing care plans, and collaborating with providers. This is a 100% remote role open to nurses with an active compact RN license. Salary range: $70,000–$90,000 plus benefits. Requires 3+ years of clinical experience, preferably in case management or managed care. BSN preferred.
Key responsibilities
- Conduct telephonic assessments to identify member needs
- Develop and implement individualized care plans
- Coordinate with healthcare providers and community resources
- Monitor member progress and adjust plans as needed
- Document all interactions in the care management system

- Active compact RN license
- 3+ years clinical experience
- Case management or managed care experience preferred
- Strong communication and organizational skills
