Job description
CVS Health is hiring a Case Manager RN for the Southeast Region. This is a remote position supporting members with chronic conditions through telephonic outreach and care coordination. Responsibilities include assessing member needs, developing care plans, coordinating with providers, and educating members on self-management. Requires active RN license, 3+ years clinical experience, and case management experience preferred. BSN preferred. Salary range $65,000-$95,000.
