Job description

The Care Manager RN is a key member of CVS Health's Special Needs Plan (SNP) care team, coordinating care for members with multiple chronic medical and behavioral health conditions and social determinants of health (SDoH) needs. 50-75% of the day is dedicated to telephonic engagement with members and care coordination. Conduct comprehensive assessments, develop individualized care plans with goals and interventions, and provide evidence-based disease management education. Collaborate with the interdisciplinary care team to address barriers to care and connect members with health plan benefits and community resources. Document care management activity meticulously in the member's electronic health record and ensure regulatory requirements are met within specified timelines. Support other Care Team members through clinical decision making and guidance.

Requires active, unrestricted RN license in New York, minimum 3+ years nursing experience, and minimum 2+ years of case management, discharge planning, or home healthcare coordination experience. ASN required; BSN preferred. Experience with Medicare/Medicaid populations, SDoH, and bilingual English-Spanish skills are preferred. Must have a private, dedicated home workspace and proficiency in Microsoft Office. Salary range $66,575–$142,576 plus bonus eligibility.

  • Medicare/Medicaid managed care experience (preferred)
