Job description
The Case Manager RN coordinates care for members with complex medical needs, working remotely to assess, plan, and implement care strategies. Collaborates with physicians, members, and families to ensure appropriate utilization of resources and adherence to treatment plans. Requires active Illinois RN license, 2+ years clinical experience, and strong communication skills. BSN preferred. Salary range $65,000-$85,000.
