Job description
As a Case Manager RN with CVS Health, you will coordinate care for Medicare members via telephonic outreach. Assess member needs, develop care plans, and connect members to community resources. Work from home with flexible schedule. Requires active RN license, 3+ years clinical experience, and case management experience preferred. Salary range $70,000-$90,000.
Key responsibilities
- Conduct telephonic assessments to identify member needs
