Job description
Field-based nurse case manager serving members in the Northern Virginia/Woodbridge area. Telephonic and in-home assessment, planning, and coordination of case management activities to evaluate members' medical needs and facilitate overall wellness. Requires up to 75% travel to member homes.
Key responsibilities
- Assess, plan, implement, and coordinate case management activities
- Conduct in-home member visits
- Develop care plans addressing complex health and social needs
- Present cases at multidisciplinary case conferences
What you bring
- Active unrestricted Virginia RN license
- 3+ years clinical RN experience
