Job description

Field-based case manager conducting in-home face-to-face assessments for members enrolled in NJ Managed Long-Term Services and Supports (MLTSS) and Dual Special Needs (D-SNP/FIDE) programs. Travel 50-75% in Atlantic, Cape May, and surrounding NJ counties. Use comprehensive assessment tools to evaluate medical needs and authorize services such as adult/pediatric medical daycare, personal care assistance, and nursing facility custodial requests. Coordinate care with members, PCPs, and interdisciplinary teams; develop care plans; attend interdisciplinary meetings; advocate for safe discharges. Document in EHR. Mentor new hires once proficient. Requires active unrestricted NJ RN license, 3+ years clinical experience, residence in Atlantic/Cape May or adjacent county, and successful completion of NJ Choice Certification (80%+ exam score). Salary $66,575–$142,576 plus bonus eligibility.
